This two-part carbonless form guides you through the essential elements of an effective report.
Before your report, use a red roller ball type pen to enter the patient’s name, date of report and reasons for consulting your office. Check the examinations you conducted and below them, the significant findings to highlight at their report.
List where you found subluxations. Fill in your recommendations, home care instructions and the scheduled date of his or her first progress examination. Rubber stamp or sticker your office information at the bottom. On the spinal illustration, draw any postural distortions, scoliosis, high shoulder or uneven hips.
In just a few minutes of preparation you’re ready to deliver a succinct and thorough patient report of findings. When completed, it might look something like this.
At the conclusion of your report, remove the top copy and place it, along with your other report documents, in their VIP Report Folder, keeping the bottom for your files.
